Immunization Requirements

Submit your immunization records through the Student Health Portal.

1. Log in using your CAS credentials
2. Choose "My Forms" and select "Immunizations"
3. Enter the dates of your MMR vaccines
4. Upload a copy of your vaccine record
5. Select "Submit." Please allow up to 24 hours for compliance to be processed.

Need a vaccine or titer test?

• MMR vaccines and titer tests (a blood test showing immunity) are available by appointment at the Student Health Center.
• If you already have MMR titer results, upload your lab report under the Upload Documents tab in the Student Health Portal.

Need a copy of your immunization records from the FLC Health Center?

If possible, try to locate your original immunization record before contacting us. If you can’t find it, you can request a copy from the Student Health Center by submitting the form below.

• Or fax to: 970-247-7621

Please allow 3–7 business days for processing. We’ll scan and email the record to you, and you’ll be responsible for forwarding it to the appropriate school, provider, or agency.

Need to send your immunization records to Fort Lewis College?

Ask your physician or previous school to complete the form below and submit your immunization documentation directly to us.

You have the legal right to opt out of MMR vaccination by filing a non-medical exemption. However, in the event of a measles, mumps, or rubella outbreak, you may be excluded from campus for up to 3 months.

To file a non-medical exemption in the Student Health Portal:

• Option 1: Upload a signed Non-Medical Exemption Form from a licensed immunizing provider, OR
• Option 2: Parents/guardians, emancipated students, or students 18 years or older have the option of obtaining a nonmedical exemption without a provider's signature by completing the Online Immunization Education Module and obtaining a Certificate of Nonmedical Exemption.
